You are required to print out and fill up this form and submit it to Olga Konstantivnona, room No.4 from 3.00pm to 5.00pm, preferably by Friday, 2nd of April 2010.

Please bring along your zachut book. She is going to check your marks and make sure they are all correct.

HOW TO FILL UP THE FORM?

• Ignore the ФИО format, write your name as it is in your passport.

• This is the serial number of your SPM certificate given by your school (not the SPM result slip).
• To those who just realized that they have not taken the certificate from their school,just give your school a call and ask them. (i'm sure they still keep it, and just ask for fun from which year is the collection. You'll be surprised ;p)
